MEDICAL RELEASE FORM
UNIVERSITY PARK, Pa. – Head coach Jeff Tambroni and the Penn State men's lacrosse team will host free youth lacrosse clinics for first through eighth graders in partnership with Centre Lacrosse. The clinics will be on September 14, 22, and 28 at Panzer Stadium from 5:30-6:30 p.m.
Please note that the clinics will be geared towards the beginner lacrosse player. The staff will be introducing and focusing on fundamental skills and small-sided drills/games throughout our clinics.
Campers will need a signed medical release form (see link at top) to be emailed to assistant coach John Haus (jgh32@psu.edu) or submitted on the day of the clinic. Participants will not be permitted to participate without a medical release form. Please complete form prior to arrival to the clinic; forms will not be available to complete at the clinic.
Please be aware of the following COVID-19 precautions.
- Medical release form to be emailed in prior to the clinic.
- Walk-in registration will NOT be available.
- NOTE: A COVID-19 test is not required to attend the clinics.
- Parents, family members, and other guests will be permitted to watch during camp within a designated area.
MASK UPDATE (as of August 8, 2021)
- Outdoor camps - Individuals not required to wear masks.
- Indoor camps - ALL INDIVIDUALS (vaccinated and unvaccinated) are required to wear a mask indoors on Penn State's campus, including while using public transportation.
Participants are strongly encouraged to bring full lacrosse equipment.
In accordance with NCAA guidelines, all Penn State University Sport Camps and Clinics are open to any and all entrants, limited only by specified number, age, grade level and/or gender of its participants.
