Important Information for Parents/Guardians

All Penn State youth programs have policies in place to ensure the safety of youth participating in our programs, activities and services is not compromised. All program staff are trained in emergency protocols and all relevant internal, external, and parental reporting requirements.

Authorization Forms for Parents

For the safety of your child, we must have on file, a written authorization/permission form from the legal parent/guardian, of all persons permitted to pick-up your child. You may add or delete names in writing at any time.

For the safety of your child, we must have on file, a written authorization/permission form from the legal parent/guardian, permitting your child to walk/bike/drive to and from the program.

Upon arrival to University Park campus during final registration, parent(s)/legal guardian(s) should plan to submit the following completed Authorization Forms to a Sport Camps representative. You may also fax either form to the Sport Camps office at 814-865-8883 or email us either form to sportcampinfo@psu.edu.

Confidentiality

Personal information about camp participants is considered confidential and may not be shared outside of camp. This includes the identity of campers, any demographic information, medical information and any other personally-identifiable data about any individual or group of campers. Also, the University cannot release names or phone numbers of other participants, not even to those who want to carpool.

In accordance with NCAA guidelines, all Penn State University Sport Camps and Clinics are open to any and all entrants, limited only by specified number, age, grade level and/or gender of its participants.
